1st - 8th Grade Application

Thank you for your interest in St. Catharine School. Following submission of the application, please email the admissions office a copy of the following documents:
- Your child’s latest report card
- Last year’s report card
- Standardized test scores
- If applicable, the IEP/service plan
Once all items have been received, St. Catharine School will review the application and make contact to schedule in-person interview for the parent and student with Administration.

Notice of Acceptance
Upon notice of acceptance, parents will be asked to complete the registration process and provide the following original documentation:
- Registration Form
- Create FACTS account
- Pay Enrollment Fees
- Schedule Parent/Guardian Meeting to provide child's:
- Original Birth Certificate
- Sacramental Certificates
- Immunization Records
- Health Record Form, due August 1
- Request for Student Transfer Records
- Transportation Form
- Request for Loan of Textbooks
