New Jersey State Law states the primary responsibility for the health of a child rests with the parent/guardian and their family physician or primary health care provider. SCS uses one form for all physicals called the Pre-participation Physical Evaluation. that is formulated by the New Jersey Department of Education.
Children that require physicals:
1. Children entering Pre-K, Kindergarten, or transferring to SCS into any other grade. This information is due by August 1 or a written note from the physician when the physical will be completed as soon as possible.
2. All children who wish to try out or participate on any athletic team. Sports physicals are good for only one year and must be submitted on an annual basis. There are three sports seasons (Fall, Winter, Spring). No child is permitted to tryout for a sport without a completed physical approved by the nurse and the principal.
N.J. State Immunization Regulations:
Immunizations must be up to date according to N.J. State law.
New Jersey State regulations require that children age eleven and entering grade six have one dose of TDAP on or after 9/1/2008 and born on or after 1/1/1997 plus one dose of Meningococcal vaccine. Proof of immunizations must be submitted before entry into sixth grade.
